Talk through your health history with Dr. Gerhart and an education specialist who can answer your questions.

Get your Colorado card
We help you submit your application to the state. Approval usually takes 1 to 3 business days.
Colorado qualifying conditions
You’ll need a diagnosis for at least one state-approved condition. Not sure if yours qualifies? Call us at (866) 846-2420 before you book.

Dr. Gerhart trained as an osteopathic physician in Parker, Colorado, and is a member of the Society of Cannabis Clinicians. He sees every patient at our Denver clinic.

Do I need an appointment, or can I walk in?
Walk-ins are welcome during open hours on Tuesdays, Thursdays, and Saturdays. Booking ahead guarantees your time and lets you upload your paperwork before you arrive.
What should I bring?
A valid ID and proof of Colorado residency. If you booked online, you can upload these in advance.
Can I get my Colorado evaluation online?
Relaxed Clarity evaluations in Colorado are in person only, so you can meet your provider face to face. Our other locations are in Westminster and Colorado Springs.
How much does the evaluation cost?
A standard medical patient evaluation is $120 for new patients and $100 for renewals. Veterans and industry patients pay $50. The state charges its own application fee of about $52 to $54.
How do I renew my card?
Book a renewal evaluation before your card expires, then upload your new certification to the CDPHE portal. We’ll remind you when it’s time and help with the state paperwork.
Can minors get a medical card in Colorado?
Yes. Patients under 18 need certifications from two licensed physicians and a parent or guardian registered as their caregiver with the CDPHE.
