If you are visiting this clinic for the first time, we ask that you call ahead at (773) 887-0420 and book an appointment before visiting in-person.
To ensure your visit goes smoothly, please follow these steps:
- In order to qualify, you must possess a qualifying condition as outlined by the Illinois Department of Public Health.
- Obtain a written certification from a physician specifying your debilitating condition, unless you are a veteran receiving health services at a VA facility. Veterans must submit one year of medical records from the VA facility where they receive services.
- Bring your Illinois Driver’s License, State ID or Passport to your appointment at Canna Care Docs to verify your residency in the State of Illinois.
- One of our state-approved practitioners will complete your evaluation. One of our education specialists will guide you through the process of the paperwork involved to get your medical cannabis certification and answer any questions you may have.
- Pay your fee by cash, credit or debit. This fee includes the doctor’s time, certification and passport photo.
- Our staff will provide support for qualified patients with their State of Illinois forms.
