Renewing Patient Instructions
Annual Review
Contact your healthcare provider to schedule a review of your condition and discuss the continued use of medical cannabis.
Attend Your Appointment
Go to your scheduled appointment to have your medical and treatment needs reassessed.
Recertification
If your provider confirms that ongoing treatment with medical cannabis is
appropriate, they will recertify your eligibility.
New Card Issued
Your provider will print a new medical cannabis card on tamper-proof
paper.
Receive Your New Card
Your provider will mail the new card to your address.
