This step-by-step guide walks you through the entire Illinois Medical Cannabis Patient Program application process, from gathering your medical records to completing your online evaluation with CMed’s licensed providers. New Patient Instructions

 

Create an Account

Navigate to Illinois Cannabis Tracking System.

Register Your Details

Enter your email address, first name, last name, username, the last four digits of your Social Security number, and date of birth, then click "Register."

Activate Your Account

Check your email for a temporary password and log into state site.

Set Your New Password

Enter the temporary password emailed to you, create a new
password, and click "Change Password."

Begin Registration

Once logged in, click "+ New MCPP Patient Registration" and
complete the application form:
- Choose if you are applying for yourself or a minor.
- Indicate whether your qualifying medical condition is terminal.
- Select the duration of your registration.
- Fill in your personal details including address, phone number, and email.

Upload Documents

Upload a state ID and proof of residency. If using a passport,
additional residency proof is needed.

Upload Your Certification

Click "+New," enter your details, and upload your physician’s
certification. Click "Save."

Process Payment

Choose the registration duration and process the payment
accordingly. Sign the form digitally and click "Save."

Await Your Card

If approved, your patient card will be mailed to you within 6 weeks.
