Confirm that your Low THC Oil Card is due for renewal (valid for 2
years).

Consult Your Physician

Visit your physician to re-assess your eligibility and have them update
your information in the registry if needed.

Physician Updates Registry

Ensure your physician re-enters or updates your information in the
DPH’s “Low THC Oil Registry.”

Notification of Renewal

You will be notified within 15 business days when your renewed card
is ready for pickup.

Renewal Pickup

Pick up your renewed card at the notified public health office. You
may need assistance in determining the closest location again.

Authorize Renewal Pickup
