Getting your Arizona medical cannabis card through the Arizona Department of Health Services (ADHS) doesn’t have to be confusing. This step-by-step guide walks you through the entire Arizona Medical Marijuana Program application process, from gathering your medical records to completing your online evaluation with CMed’s licensed providers. New Patient Instructions

 

Log In to the State Portal

Create a new account using your personal email address and credentials on the ADHS Individual Licensing Portal.

Access the Individual Portal

Once logged in, navigate to and click on the "Individual Portal."

Apply for a New License

In the portal, click on "Add a New License," then select "Marijuana" from the options provided, followed by "New Patient Application."

Prepare to Apply

Ensure you have the following documents ready in a digital format for uploading:
-A clear electronic copy of your photo identification
-A recent digital photograph of yourself
-Your completed and signed physician certification form
-Your completed and signed qualifying patient attestation
-SNAP documentation, if you are a recipient (optional)
-Proof of address, if your Arizona ID does not reflect your current address

Consent to Terms

Scroll down to the bottom of the page, read the terms carefully, and click on "Agree and Proceed."

Enter Your Patient Information

Provide the required information:
-First name, last name, date of birth, and gender
-Type of ID (driver’s license, passport, ID card, or registry ID card), the ID number, issuing state (choose AZ), and the issue date

Input Contact Details

Provide your contact information accurately:
Address (or mark if you are homeless), city, state, and ZIP code
Your phone number

Sign Electronically

Use your mouse or touchscreen to draw your signature in the provided box. This confirms that all information provided is correct and that you agree to the terms.

Review and Submit

Review your information for accuracy. Once verified, submit your application.

Pay the Application Fee

With a credit or debit card (American Express not accepted), complete the non-refundable $150 application fee. If you receive SNAP, the fee is reduced to $75.

 Submit Your Application

After payment, click the final submit button to send your application for processing.

Confirmation

Look for a confirmation email to ensure your application has been received. Keep this for your records.

 Processing and Card Issuance

The Department processes applications within 5 to 10 business days of receiving both the application and payment. Approved patients will receive an electronic medical marijuana card, valid for two years from the date of issuance.
