New Patient Instructions
Create an Account
Select 'New Patient Registration' and create a username and
password on the Missouri DPH site.
Log In
Sign in using the username and password you just created.
Start New Application
Click on 'Create New Application.'
Fill in General Information
Complete the 'General Information' tab with your details.
Enter Contact Information
Proceed to the 'Contact Information' tab and provide your contact
details.
Provide Physician and Condition Information
Fill out the 'Physician/Condition' tab. Select and attach your
physician's certification to your account.
Upload Required Documents
Upload a copy of your ID, a recent photo of yourself, and proof of
hardship (if you're applying for a reduced fee).
Submit Payment
Pay the required state fee for processing your application. The
standard fee is $25, with a reduced fee available for those who qualify
