Access the Website

Visit the Iowa OMC Application site.

Declare New Patient Status

Select "No, I am a new patient" to start a new application.
Complete the Application Form
Fill in all required fields:
- First name, last name, gender, date of birth
- Driver’s license or state ID number
- Race, ethnicity
- Phone number, email address
- Appointment type
- Permanent Iowa address: street address, city, zip code
- Mailing address, if different from the permanent address

Upload Required Documents

Upload the following documents:
- A valid photo ID
- Health care practitioner certification (ensure all pages are
included)
Apply for Fee Reduction (Optional)
If you are eligible for a fee reduction due to disability, Medicaid, SSI,
or veteran status, select the corresponding option and provide proof.
If not, proceed to the next step.

Sign and Confirm Your Application

Digitally sign your application to confirm the accuracy of all
information provided.

Proceed to Payment

The application fee is $100, or $25 if you qualify for a reduced fee.
Proceed to the payment screen and complete the payment.

Submission and Confirmation

Submit your completed application. You will receive an approval email
with your digital card attached as a PDF or notification of any
pending information needed.
