New Patient Instructions
Access the Online System
Visit the Rhode Island Cannabis Licensing website.
Create an Account
Create a username and password, and fill in the required information.
Apply for a License
Click on “Apply for License”. Select 'Medical Marijuana Cards Apply' and complete the patient details form.
Answer Application Questions
Complete the four questions located below the Patient information section.
Enter Caregiver Information (if applicable)
If someone assists with your medical care, provide their information and answer the related questions.
Enter Purchaser Information (if applicable)
If someone else is obtaining medical marijuana on your behalf, provide their information and answer the relevant questions.
Provide Identification Documents
Attach the necessary documents such as proof of residence, driver's license, or photographs as specified in the application requirements.
Provide Practitioner Information
Fill out the Practitioner Details section using the Practitioner Written Certification Form information received during your Rhode Island medical marijuana evaluation. Ensure to complete the signature section below the licensed provider's details on your RIDOH Cannabis Licensing Portal application.
Sign and Submit Application
Electronically sign the registration application to confirm your agreement with the provided information and submit the application for processing.
Allow 3 Weeks for Verification
Upon successful verification, your medical marijuana card will be mailed to the home address provided during the application process.
